Description

In multimorbid patients, the overlapping symptoms and treatment strategies result in a highly complex disease, therapy and associated consequences. For example, mobility restrictions, malnutrition, cognitive impairment, incontinence, pain and chronic wounds can develop. The high number of patients is offset by an impending shortage of GPs, which already exists in many rural regions. The FAMOUS project aims to stabilise the healthcare and home care situation in order to cover the care needs of multimorbid patients. This is achieved through needs-based support and by strengthening the patient side through the deployment of Advanced Practice Nurses (APN). As part of the project, a study will be conducted in which a group of patients will receive case-related care from an APN close to their GP. Data on the context, implementation and mechanisms of action will be collected for the process evaluation.
